This position supports the mission of the Sedgwick County Health Department by promoting the wellness of Sedgwick County residents through the Healthy Babies Program. The Community Health Worker works with high-risk pregnant and postpartum individuals to reduce infant mortality, maternal mortality, low birth weight, and prematurity. Responsibilities include community outreach, client education, screenings, resource navigation, referrals, and follow-up services to address barriers to healthy pregnancy and birth outcomes. The position collaborates with community partners, supports program data collection and documentation requirements, and participates in public health emergency response activities as assigned (Activation Level 1).

Minimum Qualifications: High school diploma or equivalent. One (1) year in Customer Service Setting.One (1) year of Community Health Worker, or Navigator experience. One (1) year of experience using Microsoft Word and Excel. Per Sedgwick County policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. Must complete Federal Emergency Management Administration (FEMA) IS 100 and 700 courses within 30 days of employment.
Preferred Qualifications: Two (2) years CHW experience, with a minimum of one (1) year as certified CHW. One (1) year health education or maternal and child health experience. One (1) year of data entry experience. Associate’s degree in business or health-related field. Completion of Community Health Worker training (Certified CHW).
